Personal Data Sheet

Local Government Unit (LGU) employee’s master data is based on CS Form No. 212 Personal Data Sheet (PDS) and other related tables that integrate into the payroll setup. Easy to generate PDS CS Form. 212 in Excel format that populates data from employee master data stored in the database.

Private employee master data is typically kept confidential and is used for HR and payroll purposes.

Notices are filed online, and paperless approval requests are automatically sent to the designated approver’s inbox.

Overtime

All overtime hours must be properly filed for approval and must be authorized and signed by the supervisor or department head before they can be accepted in the timekeeping process.

Leave Request

Employees must have available Vacation Leave (VL) credits when filing a leave with pay. The system will not process the request if there is no available leave credit balance.

Locator Slip

Upon returning from official business, the completed original copy of the Locator Slip must be converted to a PDF file and attached to the request for approval.

Temporary Shift

Employees who need to change their work shift must file a temporary shift request to override their assigned permanent schedule. This request must be approved before the shift change takes effect.

Broken Time

Employees with non-continuous work schedules must file a Broken Time Request, provided the total regular or production hours remain the same. Approval is required.

Failure IN/OUT

After the timekeeper generates the timesheet for the cut-off period, the system automatically notifies employees with invalid time logs for filing and correction.

Manpower Request

Manpower requests must be submitted for review by the department head before being forwarded to HR for approval. Once approved, the request is automatically queued for recruitment.

Work-from-home

Employees working from home must record their daily task activities and submit a report every payroll cut-off period for review and approval.
